These images look quite similar to images of a pseudomyxoma peritonei which was discussed before. It occurs most commonly with the cancers of the gastrointestinal tract and ovaries. Tumors also occur at other sites of fluid stasis including the mesenteric folds right paracolic gutter and subhepatic and subphrenic spaces. A less obvious medial paracolic gutter may be formed especially on the right side if the colon possesses a short mesentery for part of its length.

Etiologically it means a channel adjacent to the abdominal wall. The right lateral paracolic gutter runs from the superiolateral aspect of the hepatic flexure of the colon down the lateral. It is the depression between the postero lateral wall of the abdomen and the lateral margins of the ascending and descending colon. Peritoneal carcinomatosis is the most common secondary tumor affecting the peritoneal cavity.

Disease may spread from the retroperitoneum which contains the aorta to the peritoneal space along the peritoneal cavity around aortic branches which extend into the mesenteries the so called subperitoneal space 4 5. The paracolic spaces gutters are located lateral to the peritoneal reflections of the left and right sides of the colon fig 8a. In a male patient this is a very uncommon diagnosis. The inframesocolic space also contains paracolic gutters which are peritoneal recesses that are inferolateral extensions of their corresponding inframesocolic spaces on the posterior abdominal wall lateral to the ascending and descending colon respectively.

The right paracolic gutter is larger than the left and communicates freely with the right subphrenic space. Either of two grooves formed by the peritoneum and lying respectively lateral to the ascending and descending colons. It is also known as sulci paracolic and paracolic recesses. Paracolic gutters refer to open areas between the wall of the abdomen and the colon.
